Can anyone reproduce this :
open say 7 or 8 tabs, wait until all are loaded then >context menu>reload>reload all tabs - only the 6 tabs from the left reload, the 7th, 8th etc don't reload.

I just tried with 7 then 9 tabs and all reloaded. I am using KM 1.5.3 rv:1.8.1.22 on XP=SP3. What version are you using?

I traced the context menu item back to these lines in main.kmm but I don't know how to go further.

Reload all option need JavaScript enable.
Are you sure that you not use NoScript or AdBlock extension for filter these pages?
